Apply for Designation

The Fit Community application process is a thorough evaluation that can and will benefit your community in numerous unexpected ways.

All applications due for designation must be submitted to Active Living by Design by 5:00 p.m. on March 18, 2011. Late applications will not be accepted. Click here for a complete Timeline of the application process.

 
GET STARTED
  1. Assemble a diverse partnership, including community leaders and members of target population.
  2. Download the Request for Applications (RFA) and make copies; each partner should become familiar with the Active Living by Design 5P Model, the criteria used to evaluate all Fit Community applications.
  3. Register to participate in the conference call for applicants (see RFA for details). A recording of the conference call can also be downloaded - audio file / presentation..
  4. Work together to complete the Fit Community Designation Self-Assessment and other parts of the Designation Application.
